Therapeutic Target for Patients with Type 2 Diabetes

A recent study published in the journal Nature Communications showed a therapeutic target for the preservation and regeneration of beta cells, the cells in the pancreas that produce and distribute insulin. The study was led by researchers from the Diabetes, Obesity, and Metabolism Institute at Mount Sinai. The team found a molecular mechanism that appeared to be involved in beta cell preservation and regeneration through carbohydrate response-element binding protein (ChREBP).

The researchers found that the production of a hyperactive isoform of the ChREBP protein is necessary to produce new beta cells in response to an increased demand for insulin in the body, primarily due to a high-fat diet or significant glucose exposure. However, they also revealed that prolonged exposure to glucose metabolism could result in a vicious cycle of ChREBP overproduction, resulting in glucose toxicity and beta cell apoptosis.

Dr. Donald Scott, PhD., Professor of Medicine at Mount Sinai, and a member of DOMI stated that traditionally, ChREBPb, a form of ChREBP, was thought to be a mediator of glucose toxicity. But in the study, the researchers noticed another form of ChREBP, ChREBPa, appeared to protect beta cells. He added that they developed these isoforms independently and found that ChREBPb played a vital role in the gradual destruction of beta cells, resulting in hyperglycemia and glucose toxicity. He further stated that removing ChREBPb or pharmacologically counteractive agents of the protein could mitigate the effects of glucose toxicity and protect beta cells. This is an exciting discovery as it creates an opportunity to develop therapeutic agents that target the known molecular mechanism, effectively blocking ChREBPb production and preserving beta cell mass.
